A responsibility statement describes what you were assigned to do. An outcome statement describes what changed because you did it. Recruiters and hiring managers scan for the second kind — it's the difference between "this person had this job" and "this person produced this result," and only one of those predicts whether they'll produce a result for you.
The three-part structure
The most reliable bullet structure is: what you did, specifically — the mechanism or method — and the measurable result. You don't need all three in every bullet, but a resume where most bullets have zero of the three reads as a job description restated in past tense, not a track record.

What to do when you don't have a clean number
Not every task produces a metric, and fabricating one is worse than not having it. When there's genuinely no number, scale or scope still works: team size, systems touched, frequency, or the specific problem solved. "Fixed a bug" is weak; "diagnosed a memory leak that had caused three production incidents in the prior month" carries real signal without inventing a statistic.
